Cleveland Guardians vs Tampa Bay Rays Picks and Predictions for Monday October 19 2026

By: Kim Smith Published 10/18/2026, 09:45 PM ET

Cleveland had better find some runs, and fast. The Guardians scored once Sunday, lost 3-1, and now they’re headed back to Tampa Bay needing two wins to reach the World Series. The Rays aren’t exactly beating the cover off the baseball, but they’ve won two straight and have the Guardians right where they want them. One more win and Tampa Bay gets Milwaukee. Cleveland gets to start thinking about spring training if it loses.

Game 6 of the American League Championship Series is Monday, October 19, 2026, at Tropicana Field in St. Petersburg, Florida. First pitch is scheduled for 8:08 PM ET on TBS, truTV, and HBO Max. Tampa Bay is listed around -136, Cleveland +113, with the total at seven runs. The Rays lead the series 3-2, and there’s no weather to worry about with the game being played indoors. Starting Pitchers

Tanner Bibee is the projected starter for Cleveland, with Freddy Peralta expected to go for Tampa Bay. Neither had been officially confirmed on MLB’s game page when this preview was written.

Bibee is a bit of a question mark. His postseason work has been limited, and Cleveland used him in relief during Game 4 of the ALDS rather than giving him a normal start. The Guardians need innings Monday, not another night of getting four outs here and five outs there from different pitchers. If Bibee can’t get through the middle innings, Cleveland could find itself asking a tired bullpen to save the season.

Peralta gives Tampa Bay a more comfortable situation. He worked six scoreless innings against Cleveland in Game 2, allowing four hits and striking out six. The Guardians didn’t do much against him. They managed one run in the entire game and lost 3-1. Peralta also pitched in relief during Game 4, so the Rays will need to decide how much they can reasonably ask from him Monday. That’s worth watching. He’s been good against Cleveland, but October isn’t the time to pretend a pitcher’s workload doesn’t matter.

Cleveland Guardians Are Running Out Of Chances

Cleveland went 85-77 during the regular season and has already survived one elimination series this October. The Guardians won Game 1 of this series 7-3 and took Game 3 by a 3-2 score. Since then, it’s been rough. They lost 7-3 Saturday and 3-1 Sunday. That’s four runs in two games, and now they’re facing elimination on the road. The Guardians had chances Sunday, but they couldn’t put much together against Tampa Bay’s pitching. They’re not getting another chance after Monday if this keeps up.

Cleveland hit .239 during the regular season with a .380 slugging percentage and 162 home runs. That’s not an offense built to win by hitting three homers every night. The Guardians need baserunners, productive outs, and a few timely hits. They scored seven in Game 1, but they’ve managed five total runs in their three losses. Tampa Bay has been making them earn everything. Cleveland’s pitching has generally kept the club competitive, but giving up seven in Game 4 put the offense in a hole it couldn’t climb out of. The Guardians can’t afford another early deficit.

The bullpen has done a lot of heavy lifting this postseason, and that’s going to matter again Monday. Cleveland used several arms in Game 4, then had to get through Game 5 after its starter lasted only two innings. That’s a lousy way to head into an elimination game. The Guardians need their projected starter to give them something substantial. Four decent innings might not be enough. Tampa Bay has shown it can wait out Cleveland’s pitching changes and find a couple of runs late. The Guardians can’t keep asking the relief corps to cover half the game.

Tampa Bay Rays Are One Win From The World Series

Tampa Bay went 98-64 in the regular season and has been tough to beat at home. The Rays won 58 home games, and now they get two chances to finish Cleveland off at Tropicana Field. They didn’t need much offense Sunday. Three runs were enough because the pitching was excellent. The Rays scored early, then let their pitchers handle the rest. That’s a pretty comfortable way to win a playoff game. Tampa Bay also took Game 4 by a 7-3 score after Cleveland had grabbed a 2-1 series lead. The Rays have responded when they needed to.

Tampa Bay’s pitching was one of its biggest strengths all year, finishing with a 3.51 team ERA. That showed up again Sunday. Cleveland managed one run, and the Rays never had to play catch-up. The offense has been inconsistent in October, but Tampa Bay doesn’t need ten hits every night when it’s getting that kind of pitching. The Rays scored seven Saturday and three Sunday. Different kinds of wins, same result. They’ve also been good at taking advantage of mistakes. Cleveland has given them a few extra outs during this series, and Tampa Bay has made those hurt.

The Rays have some decisions to make with their bullpen, especially after using several important relievers over the weekend. They got seven innings from their starter Sunday, which helped a lot. Monday could be different if Peralta isn’t able to work deep after his recent relief appearance. Tampa Bay has enough arms to piece things together, but the Rays would rather not turn Game 6 into a parade of relievers by the fourth inning. Still, they’re in a better spot than Cleveland. They have the series lead, they’re home, and they don’t have to win Monday to stay alive.

Cleveland Guardians vs Tampa Bay Rays Picks and Predictions

Tampa Bay Rays Moneyline (-136)

I’m taking Tampa Bay at -136. Cleveland has been living on the edge all October, and eventually that catches up with a team. The Guardians have scored four runs in their last two games, and now they’re facing a pitcher who shut them out for six innings in Game 2. Peralta’s recent workload is the part I don’t like. If he can’t go very far, Tampa Bay will have to get creative. But Cleveland’s pitching situation isn’t any prettier. The Guardians had to cover seven innings after their starter got knocked out Sunday, and that bullpen has been working plenty. Tampa Bay has won two straight, gets to play at home, and has been better at turning its chances into runs. I’ll lay the price.

Under 7 Runs (-110)

Seven is a tight number, but I’m taking the under. Three of the five games in this series have finished with five combined runs or fewer. Sunday’s game ended 3-1, and Game 2 was also 3-1. Cleveland has had trouble doing much against Tampa Bay’s pitching, and the Rays don’t need to swing for the fences when they get an early lead. The concern is the starting pitching situation. If Bibee doesn’t last or Peralta is limited, this could turn into a bullpen game and get ugly. We’ve already seen Tampa Bay score seven in Game 4. But with a trip to the World Series on the line, I expect both clubs to use their best available arms and keep the game close. Something around 4-2 Tampa Bay makes sense.

Final Picks: Tampa Bay Rays Moneyline (-136) and Under 7 Runs (-110)
